Three ways facilities credit concurrent and group time

MethodWhat you’re credited for a 60-minute session with 2 patientsFormula
Clock minutes60session minutes
Split30session minutes ÷ patients
Patient minutes120session minutes × patients

Productivity % = (one-on-one minutes + credited minutes) ÷ paid minutes × 100

Worked example: SNF PT

Paid 480 minutes. 330 one-on-one minutes, plus one 60-minute concurrent session with 2 patients.

Crediting methodCredited minutesProductivity
Clock minutes330 + 60 = 39081.3%
Split330 + 30 = 36075.0%
Patient minutes330 + 120 = 45093.8%

Same day, same work, an 18.8-point spread. Get your facility’s method in writing before you plan around it.

Medicare rules you need to know

Crediting for productivity is an employer decision. Medicare billing rules are separate, and they differ by payment system.

SNF Part A (PDPM)

  • Concurrent therapy: one therapist treating two patients doing different activities
  • Group therapy: one therapist treating a small group doing the same or similar activities
  • Combined limit: concurrent and group therapy together may not exceed 25% of therapy minutes, applied separately for each discipline. Compliance is tracked on each resident’s PPS discharge assessment (item O0425).

Outpatient Part B

  • CMS billing guidance says that when a therapist divides attention between patients, gives brief intermittent contact, or gives the same instructions to two or more patients at once, each patient is billed as group therapy (97150), which is untimed.
  • If one-on-one contact is tracked separately, those minutes can be billed as individual timed therapy instead.
  • Timed codes and 97150 can’t be billed in the same 15-minute period.
  • For SLPs, group treatment is reported with 92508.

Frequently asked questions

Does concurrent therapy count double for productivity?

Only if your facility uses the patient-minutes method. Others credit clock minutes or split the time.

What is the PDPM 25% rule?

Under PDPM, concurrent and group therapy combined can’t exceed 25% of therapy minutes for each discipline.

Is concurrent therapy allowed under Medicare Part B?

Part B doesn’t pay for “concurrent” as its own category. Per CMS guidance, simultaneous treatment without one-on-one contact is billed as group therapy (97150).

Who decides how concurrent minutes count toward my productivity?

Your employer. Medicare rules govern billing, not productivity crediting.
