Utilization vs productivity
| Productivity | Utilization | |
|---|---|---|
| Question | How much of my paid time was billable? | How much of my scheduled patient time was billable? |
| Formula | billable ÷ paid minutes × 100 | billable ÷ scheduled patient-care minutes × 100 |
| What it reveals | Overall workload balance | Scheduling gaps, cancellations, no-shows |
Employers use these terms differently, and some use “utilization” to mean what others call productivity. Ask for your facility’s definition.
Worked example: outpatient OT
Rosa’s schedule had 450 minutes of booked patient care. She delivered 396 billable minutes.
- Utilization: 396 ÷ 450 = 88%
- If two 45-minute patients had cancelled and the slots stayed empty: (396 − 90) ÷ 450 = 68%
That 20-point swing comes from the schedule, not from Rosa’s work.

How to use utilization data
- Track cancellations separately. It separates scheduling problems from clinical workload.
- Compare by day of week. Some clinics see more cancellations on particular days.
- Use it to argue for waitlists or double-booking policies, where clinically appropriate.
Frequently asked questions
What is a good therapy utilization rate?
There’s no published national standard. Compare your own trend over time and against your clinic’s target.
Is utilization the same as productivity?
Not always. Productivity usually uses paid time as the base, while utilization uses scheduled patient-care time. Some employers use the words interchangeably.
Do cancellations lower utilization?
Yes. Unfilled cancelled time stays in the scheduled minutes but adds nothing to billable minutes.
