Productivity vs Billable Time

Billable time is the minutes of treatment that can be billed to a payer. Productivity is a ratio: billable (or productive) minutes divided by paid minutes. Billable time is one number. Productivity is that number measured against your workday. And the minutes your employer counts as “productive” aren’t always the same as the units that end up on a claim.

Three terms that get mixed up

TermWhat it meansExample
Billable minutesTreatment minutes that qualify for billing53 minutes of timed codes
Billed unitsWhat goes on the claim after unit rules53 minutes = 4 units under the 8-minute rule
Productive minutesWhat your employer counts in the productivity formulaOften billable minutes; sometimes includes co-treats or documentation

Why don’t billed units match my minutes?

Medicare’s 8-minute rule turns total timed minutes into 15-minute units with ranges. 53 minutes and 67 minutes both bill 4 units. So two sessions with very different minutes can bill the same.

Most employers measure productivity in actual minutes, not unit equivalents. Ask which yours uses. A system that credits 4 units as 60 minutes would show different productivity than one that uses the 53 minutes you actually spent.

What about untimed codes?

Codes like 92507 (speech treatment) and evaluations are billed as one unit no matter how long they take. Your productivity system still needs to know how many minutes you spent. Most use actual minutes on the clock.

Is all billable time productive time?

Usually, but not always. Common differences:

  • Co-treatment: payer rules vary. Under Medicare Part B, two therapists treating at the same time generally split the billable units rather than both billing the full session. Your employer may still credit both therapists fully, split the time or credit one.
  • Concurrent and group: billed differently by payer; credited differently by employer (see the concurrent therapy calculator).
  • Documentation: usually not billable. ASHA’s 2025 survey found 8% of SLPs could count documentation when the patient wasn’t present.
