Productivity vs Utilization

Productivity compares your billable minutes with your paid minutes. Utilization compares them with your scheduled patient-care minutes. Productivity tells you how much of your workday was billable. Utilization tells you how much of your booked schedule turned into billable care.

Same 396 billable minutes shown against two bases: 480 paid minutes gives 82.5 percent productivity, 450 scheduled minutes gives 88 percent utilization
Same minutes, two different bases

Worked example: one day, two numbers

An outpatient OT is paid for 480 minutes. The schedule had 450 minutes booked. She delivered 396 billable minutes.

  • Productivity: 396 ÷ 480 = 82.5%
  • Utilization: 396 ÷ 450 = 88%

Now two 45-minute patients cancel late and the slots stay empty:

  • Productivity: 306 ÷ 480 = 63.8%
  • Utilization: 306 ÷ 450 = 68%

Both drop. But utilization makes the cause clear: the booked time was there, and it didn’t turn into care.

Why does the difference matter in a review?

If your productivity is low, the first question is why. Utilization helps answer it:

ProductivityUtilizationLikely story
LowLowCancellations and gaps in the schedule
LowHighNot enough patients booked, or too many non-billable duties
HighHighFull schedule, patients showing up

Bring both numbers to a performance conversation. “My utilization was 68% this week because of 6 late cancellations” points to a fix (a waitlist, reminders) that isn’t about working faster.

Do all employers define these the same way?

No. Some employers use “utilization” to mean what this page calls productivity. Others use “productivity” for visits per hour. Always ask for the written formula.

Frequently asked questions

Is utilization higher than productivity?

Usually, because scheduled patient time is smaller than paid time. It isn’t guaranteed.

Which is better for measuring therapists?

They measure different things. Productivity reflects the whole workday. Utilization reflects how well the schedule held up.

How do I calculate utilization?

Billable minutes ÷ scheduled patient-care minutes × 100. Use the therapy utilization calculator.
