Home Health Therapy Productivity Benchmarks

Home health is the one setting where a productivity percentage is the exception, not the rule. Only 32% of home health SLPs reported a percentage requirement in ASHA’s 2025 survey. Agencies may instead measure visits per week or weighted “points.” APTA’s practice survey found home care PTs completed a median of 25 to 28 visits per week.

Three ways agencies measure home health productivity

MethodHow it worksWhat to ask
Visits per weekA set number of completed visitsDo evaluations, recerts and discharges count as more than one visit?
Weighted pointsEach visit type gets a point value; you hit a weekly point targetWhat’s each visit type worth? Is it in writing?
Percentage of paid timeBillable or visit time ÷ paid timeIs drive time paid? Is it in the bottom of the fraction?

Point systems vary a lot between agencies. A visit worth 1.5 points at one agency may be worth 2 at another, so compare offers carefully.

Turning a weekly visit target into a daily plan

Weekly targetPer day (5-day week)Per day (4-day week)
25 visits56.25
28 visits5.67
30 visits67.5

Late-week cancellations are the hardest to recover from, so front-load evaluations and longer visits early in the week when you can.

Why home health is different

  • Travel time: driving between homes is real work, but it’s rarely “productive” in a percentage system
  • Visit-type variety: starts of care, recertifications and discharges take far more time and documentation than routine visits
  • Cancellations you can’t fill: an empty slot across town usually can’t be replaced
  • Documentation in the car or at home: if you’re hourly, unpaid off-the-clock documentation is a wage issue under the Fair Labor Standards Act, as ASHA points out

Frequently asked questions

How many visits does a home health PT do per week?

APTA’s 2016–2017 survey found a median of 25 to 28 visits a week for home care PTs, depending on time spent in direct care.

Do home health therapists have a productivity percentage?

Some do, but most home health SLPs don’t: only 32% reported one in ASHA’s 2025 survey. Agencies may use visit counts or point systems instead.

Is drive time counted in home health productivity?

It depends on the agency and on whether you’re paid per visit or per hour. Ask before you accept a position.

What is a productivity point system in home health?

A scoring system where each visit type has a value, for example a routine visit worth one point and an evaluation worth more. You’re measured against a weekly point total set by your agency.
