A Between-Session Transition Routine for Therapists
The minutes between patients are where many therapy days quietly fall apart. A few extra minutes per transition, nine or ten times a day, can be most of your non-billable time. A short, repeatable routine helps you close one session cleanly and start the next one ready.

How much time do transitions really take?
| Transitions a day | Minutes each | Daily total | Share of 72 non-billable minutes at 85% |
|---|---|---|---|
| 9 | 3 | 27 | 38% |
| 9 | 5 | 45 | 63% |
| 9 | 7 | 63 | 88% |
Going from 5 minutes to 3 minutes per transition frees 18 minutes a day, enough for most of a short note or a real lunch.
What’s a good 3-minute routine?

| Step | What you do | Time |
|---|---|---|
| 1. Close | Write the key points of the note: what you did, how they responded, the plan | 60 sec |
| 2. Reset | Wipe down equipment, put things back, set up for the next patient | 45 sec |
| 3. Check | Read the next patient’s last note and today’s goal | 30 sec |
| 4. Breathe | Water, a stretch, a few slow breaths | 45 sec |
The order matters. Close the note first while details are fresh. If you leave it until the end of the day, it takes longer and you remember less.
How do you make the routine stick?
- Use the same order every time. It becomes automatic in a week or two.
- Prep the night before. Equipment you’ll need first thing, sorted.
- Keep a short template for the “close” note, then complete the full note in your planned documentation block.
- Group patients by location where you can to shorten walking time.
Frequently asked questions
Is transition time billable?
Generally not. Setting up equipment or walking between patients isn’t direct treatment.
Should I write the full note between sessions?
If your setting allows it and you can do it accurately, yes. Otherwise, capture key points and finish in a planned block. See concurrent documentation.
How do I track my transition time?
Note your end time for one patient and start time for the next for a few days. The gaps add up fast.
